Field trip permission slip template
A one-page slip with the trip details in a table and a detachable return stub below: a permission checkbox, a medical authorization checkbox, and a pre-filled emergency contact line. Print a personalized field trip permission form for the whole class in one mail merge run.

A permission slip has two jobs on one page: tell the parent everything about the trip, and come back signed. The letter half handles the first, with a trip details table covering destination, date, departure and return times, homeroom, and cost per student, plus a quiet sentence inviting families to raise payment difficulty in confidence with the office. Below a dashed detach line sits the return stub: a permission checkbox, a separate emergency medical treatment authorization, the emergency contact printed from your records, and a signature and date line.
Because the student’s name is a mail merge field on both halves, every stub comes back pre-labeled; nobody deciphers a hurried signature to work out whose slip it is. And because the emergency contact lines print what registration has on file, parents correct what is outdated instead of recalling numbers at the kitchen table.
These templates are general information, not legal advice. What a consent form must say, and what it can and cannot cover, varies by region and district; have your administration review the wording before the first trip of the year.
What is in the Word document
More than a block of text an AI chat can improvise: a complete, formatted document built for school offices that use it for real.
- A letter half with a trip details table: destination, date, departure and return times, homeroom, and cost per student
- A cost sentence that invites families to raise payment difficulty in confidence with the office
- A detachable return stub under a dashed cut line, pre-labeled with the student and homeroom
- Two separate checkboxes: trip permission and emergency medical treatment authorization
- An emergency contact line printed from your records, plus a parent signature and date line
- Ten mail merge fields, so one class-list row fills the letter and its stub

Before it goes home
- 1.Confirm departure and return times against the actual bus booking before generating; the slip is the version parents will plan pickup around.
- 2.Set the return deadline with enough margin to chase missing stubs; the day before the trip is a deadline in name only.
- 3.Pre-fill the emergency contacts from your newest registration export, and treat a stub returned with corrections as a records update, not just trip paperwork.
- 4.The medical authorization is a separate checkbox a parent can decline; decide before trip day how staff will handle students whose box came back unchecked.
- 5.Make sure the printed cost matches what the office will actually collect, and how; the slip says payment comes back with the stub.
When to use this template
Class field trips
The class list becomes a personalized slip per student, and the stubs come back pre-labeled for a tick-box check against the roster.
Whole-grade outings
Multiple homerooms in one run: the homeroom field on the stub keeps every returned slip sortable back to its class.
Clubs and teams
Away games, competitions, and excursions reuse the same consent-plus-emergency-contact pattern with a different destination and cost.
Generate it for the whole class at once
One document, one class list, one personalized copy per student. That is what MailMergic is for.
