Performance improvement plan template

A structured PIP with an employee and plan details table, clear success criteria, and a receipt-only acknowledgment signature. Copy it, download the Word document, or send a personalized plan to every employee on the list at once.

Rendered performance improvement plan: letterhead, plan title, and a details table with plan start and review dates

A performance improvement plan (PIP) documents specific performance concerns, the improvement expected, a support plan, and a review date, giving an employee a clear and fair path to turn performance around. Managers typically issue one after informal coaching has not resolved a documented, ongoing concern.

HR teams that support many managers often need to issue several PIPs with the same structure but different concerns, criteria, and dates. Upload this plan as a Word template to MailMergic, fill a spreadsheet with the details for each employee, and generate one personalized, consistently formatted plan per row.

What is inside the Word download

More than a block of text an AI chat can improvise: a complete, formatted document built for HR teams who send it for real.

  • An Employee and Plan Details table with the manager, plan start date, and review date up front.
  • Five numbered clauses: performance concerns, success criteria, support provided, review schedule, and consequences.
  • A receipt-only acknowledgment signature, so signing does not read as agreeing with the assessment.
  • Native Word merge fields for the concerns, criteria, and support text, wired for bulk issuance by HR.

Performance improvement plan FAQ

Is a PIP required before termination?
It depends on your company's policy and, in some cases, your jurisdiction; a PIP is not universally required by law before termination, but many employers use one to document a fair opportunity to improve and to support the eventual decision if performance does not improve.
Does signing a PIP mean the employee agrees with it?
No. This template's acknowledgment line makes clear that a signature confirms receipt of the plan, not agreement with the assessment it describes.
How long should a performance improvement plan last?
There is no fixed legal length; many plans run 30, 60, or 90 days, long enough to show a real trend against the stated success criteria. Set the review date to match how quickly progress can reasonably be measured in the role.
Can I send PIPs to multiple employees at once?
Yes. Upload this plan as a Word template to MailMergic, put each employee's concerns, criteria, and dates in a spreadsheet row, and run a mail merge to generate a consistently formatted plan for each one.
